Liability:

• I acknowledge the risk of potential serious injury associated with participation in mixed martial arts; thus, by signing this waiver I warrant that I am physically and medically sound to participate in mixed martial arts exercise and have voluntarily elected to do so. In acknowledging this, I will not hold any staff, owners or volunteers of DRILICH COMBAT ACADEMY PTY LTD liable for any personal injury that arises from the act of participating in mixed martial arts exercise.
• I acknowledge that whilst on DRILICH COMBAT ACADEMY PTY LTD premises, both my property and my person are to be held at my own risk. I will not hold any staff, owners or volunteers DRILICH COMBAT ACADEMY PTY LTD liable for any personal injury or loss of property occurring on DRILICH COMBAT ACADEMY PTY LTD premises. 
• I acknowledge that DRILICH COMBAT ACADEMY PTY LTD will inform my emergency contact {contact_name} shall I require medical attention and I allow DRILICH COMBAT ACADEMY PTY LTD to act on my behalf should I require serious medical attention and will not hold DRILICH COMBAT ACADEMY PTY LTD liable for any hospitalisation and/or ambulance costs that may accrue. 
• I acknowledge that I will use the Sauna in accordance with the DCA "Sauna Rules" and understand that DRILICH COMBAT ACADEMY PTY LTD is not liable for any personal injury sustained as a result of Sauna use. 
• I acknowledge that I will use the Ice Bath in accordance with the DCA "Ice Bath Rules" and understand that DRILICH COMBAT ACADEMY PTY LTD is not liable for any personal injury sustained as a result of Ice Bath use. 

Membership: 

1. I am required to give a minimum of seven days written notice prior to cancellation of membership by email or through the website enquiries form. If a membership is cancelled and reinstated, the new membership will be charged at the membership rate advertised at the time of reinstatement, regardless of previous membership rate. 
2. All regular payments must be made on time and to the sum of the agreed amount between DRILICH COMBAT ACADEMY PTY LTD and myself.
3. There will be a maximum membership freeze time of three months unless valid reasoning is provided including compassionate and medical reasons (must provide a doctor's certificate). Membership that are frozen beyond three months are subject to cancellation. Once the membership is cancelled and if the member chooses to reinstate their membership, the new membership will be charged at the membership rate advertised at the time of reinstatement, regardless of previous membership rate. 

Media: 

• I allow my photographic image and/or videographic image to be used in the promotion of DRILICH COMBAT ACADEMY PTY LTD and Directors in the media, governing mixed martial arts bodies, mixed martial arts related publications, websites and social media when deemed appropriate. 

Anti-social behaviour/defamation: 

• I understand that DRILICH COMBAT ACADEMY PTY LTD has a no bullying policy. I acknowledge that DRILICH COMBAT ACADEMY PTY LTD reserves the right to remove any one from their operating premises that: disobeys any rules, laws, instructions; who is a danger to themselves or others; who causes any damage to DRILICH COMBAT ACADEMY PTY LTD property or any member/visitor of DRILICH COMBAT ACADEMY PTY LTD property; participates in any anti-social behaviours such as bullying, assault or consumption of illicit substances. 
• I acknowledge that I will not post false reviews or statements about mine or any persons experience at DRILICH COMBAT ACADEMY PTY LTD, or experiences with any directors/coaching staff.
